Clinically relevant 11-oxyandrogens are important biomarkers in reproductive and metabolic diseases. Growing evidence has linked four adrenal-derived androgens to conditions including congenital adrenal hyperplasia (CAH), 21-hydroxylase deficiency (21-OHD) and polycystic ovary syndrome (PCOS).

At WRIB 2026, Pharmaron scientist, Tharun Ponduru, presented a novel multiplexed LC-MS/MS method for simultaneous quantification of all four 11-oxyandrogens in human serum. The method addresses real analytical challenges that have limited routine measurement of these biomarkers until now.

Difficulty in Quantifying Clinically Relevant 11-Oxyandrogens

Measuring these four androgens simultaneously is not straightforward due to physiological concentration ranges and structural similarity. Multiplexed LC-MS/MS assays for 11-oxyandrogens are not yet routinely available, which is a gap this case study addresses.

Without reliable simultaneous quantitation, researchers are left with incomplete data lacking the sensitivity needed for low concentration patient populations.

Our Multiplexed LC-MS/MS Method

This case study’s method pairs supported liquid extraction (SLE) with LC-MS/MS to achieve simultaneous quantification of all four 11-oxyandrogens from a single human serum sample. Through different techniques, our team was able to achieve chromatographic separation and linearity across all four androgens.
